The Children's Health Service 0-19, alongside others support the delivery of the Healthy Child Programme across Lincolnshire for Children, Young People and their families (aged 019 years) and up to the age of 25 years for Young People with Special Educational Needs and/or disabilities (SEND).
The CYPN Team is small and close-knit who work across Lincolnshire. They work in collaboration within the team to ensure that children, young people and families can access timely expert advice and intervention on specific health issues when needed. The CYPN team work with the health visiting teams in Childrens Health, Children's Services (Children's Centres, Family Help, Social Care and Family Hubs) and other agencies to improve outcomes and reduce health inequalities
