Integrated Health Team 9 is looking to recruit an enthusiastic, dynamic and motivated Band 6 District Nurse/Case Manager to work within the Integrated Health Team based at Stratford, Hospital. This is a community team which covers the town of Stratford, and surrounding villages.
The team strive to provide patient centred care, and actively encourage prevention and self-management, therefore the applicant will be required to work closely with the multi-disciplinary team, and maintain relationships with our GP's.
The successful candidate will need to be flexible, hardworking and organised, with at least 2 years community nursing experience. The candidate will be able to demonstrate their leadership behaviours and skills in managing the complex care of long term conditions and end of life patients, including Continuing Healthcare assessments. You will be responsible for a team of clinicians and have direct responsibility for a defined caseload of patients supported by colleagues, the community matron and professional lead.
The applicant will hold a mentoring and assessing qualification, be a community nurse prescriber and hold a Specialist Practitioner, Long Term Conditions degree or currently working towards.

The District Nurse is responsible for the assessment and management of the health needs of an identified population and caseload, focusing on public health, preventative care, and admission avoidance. They will support the team in implementation and evaluation of programmes of care within a range of target groups including preventative deterioration of complex health, health inequalities, social prescribing and signposting, management of comorbidities and supporting the prevention of an urgent care need.
The District Nurse Specialist Practitioner will clinically supervise and provide support to regulated and unregulated team members with career development, clinical capabilities and behaviours, education and supervision. They will assist the Professional Lead with recruitment and retention, support the wellbeing of the team and improve quality outcomes and budget management as well as implementing service improvement schemes and supporting in pathway development.
