Job Overview
This is an exciting role funded to primarily focus on Treating Tobacco Dependency (TTD) amongst in-patients and facilitate access to Nicotine Replacement Therapy (NRT).
As a Health Coach you will work with patients to provide advice and information on the impact of smoking on their health and medical treatment, and use behaviour change interventions to motivate patients to consider the use of NRT. Health Coaches work holistically to address lifestyle choices and health behaviours.
Main duties of the job
The Treating Tobacco Dependency service falls within the Trust’s Health Inequalities Programme. You will champion Personalised Care principles in clinical settings and amongst clinical colleagues and focus on ‘what matters most to patients’, work holistically and in a person-centred way, while identifying and tackling the barriers patients face when trying to maintain positive behaviour change. You will also signpost patients to community services when appropriate.

Keep accurate records and assist with monitoring and reporting requirements
Support the Trust’s Health Inequalities Programme and BSW Health Inequalities Strategy
Promote Personalised Care across the Trust

Qualifications
Essential criteria
- Educated to degree level or equivalent level of experience in health and social care.
- Proven experience of working with resilience with people who have care or support needs.
- Confident in the use of the Microsoft suite of applications
- Excellent interpersonal skills, which can be tailored to meet the needs of different audiences
Desirable criteria
- Experience of providing health-related interventions (i.e. smoking cessation, diabetes)
- Health Coach Qualification or equivalent experience in a Personalised Care role.
