An exciting opportunity has arisen within Lichfield & Tamworth Community D2A Team for a Level 2 Registered Social Worker (Band 6).
You will be part of the South Discharge to Assess Service and will work closely with multi-disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists and the Home First team.
As a Level 2 Social Worker, you will take a more autonomous and complex role in supporting the delivery of the personalisation and social inclusion agenda, in line with the principles of the Care Act 2014. We are looking for an experienced, enthusiastic, proactive and innovative individual who possesses excellent communication and organisational skills, alongside a strong working knowledge of the Care Act, safeguarding, and strength-based approaches.
This is a dynamic post that requires you to work closely with multi-disciplinary teams and partner agencies in addressing a range of complex care and support issues. You will play a key role in enabling individuals to remain in their own home through the promotion of independence, preventing unnecessary hospital admissions, and improving the health and wellbeing of citizens. You will also be expected to support and guide less experienced staff and contribute to service development.

You will have direct support from the Practice Lead and colleagues as well providing peer support to your team.
Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
