The G DOC Locality Pharmacy Lead delivers safe and effective clinical leadership to the pharmacy team across the Forest of Dean Locality. The role provides a key link into clinical and operational leadership within G DOC, bringing pharmacy expertise and a collaborative approach. The role will proactively lead the pharmacy team to ensure delivery of a service that is fit for the future and adaptable, and supporting readiness for neighbourhood health working.
Working with clinical and operation leadership the postholder will drive quality improvement, safety, sustainability, performance and governance across the pharmacy and wider clinical teams.
Hours: Flexible - Ideally 24 Hours a week
Salary: £70,000 WTE
Main Location: Dean Medical Practice - Coleford. With travel to all Forest of Dean practice sites.
Clinical Leadership & Delivery
Provide Clinical Leadership to the pharmacy teams across the locality within G DOC, including clinical pharmacists, pharmacy technicians, dispensing teams and prescription clerks as applicable.
Support clinical leadership across G DOC, linking into appropriate clinical and operational leadership structures within G DOC.
Support implementation and delivery of Neighbourhood Health working as applicable to the Pharmacy teams, working closely with the G DOC Clinical Director, Head of Clinical Governance and Operational Leadership. This may include supporting, leading and/or facilitating MDT meetings.
Close liaison with G DOC clinical and operational leadership to direct pharmacy team working and integration across G DOC.
Act as a link to the PCN within the locality on medicine management and optimisation.
Lead on implementing national, regional and local contract changes and guidance as applicable to the pharmacy teams, supporting organisational resilience and proactive solutions.
Support wider organisational resilience as relevant to the pharmacy teams across G DOC as appropriate, which may include some working in and with other locality areas.
Lead and co-ordinate on long term condition management as applicable to the pharmacy teams, working collaboratively with clinical teams across the organisation.
Champion health equity for the populations we serve, seeking to improve health outcomes and efficiency of the practice team by ensuring direct, accessible and timely medicines expertise.
Proactively lead the management of people with complex polypharmacy.
To support and work with G DOC GP practices within the locality. In this role the Locality Pharmacy Lead will be supported by the relevant locality GP Leads, G DOC Clinical Director and Head of Clinical Governance and work as part of a multidisciplinary team. They will work autonomously and travel between surgeries to ensure that the population of the locality has equitable access to their service.
They will provide clinical support to care homes within the PCN and/or ensure this is in place.
Medicines Management and Optimisation
Oversight of medicines optimisation management within the locality and centrally within G DOC, linking in as appropriate to other system partners.
Contribute and prepare reports or reporting frameworks as appropriate to the role.
Proactively identify and address medicine matters, safety alerts and other issues as relevant to safe and effective medicines management.
Support digital elements of medicines management across the locality, liaising as necessary. Areas may include SystmOne, Ardens, Radar and digital AI tools.
Clinical Governance and assurance
Assist with investigation, reporting and actions resulting from Patient Safety Incidents (Significant Event Analysis) and alerts as applicable to the pharmacy teams and/or medicine management.
Oversight for quality, improvement, clinical governance and transformation as applicable to the pharmacy teams in the locality, working closely with G DOC clinical and operational leadership structures.
Escalate matters as appropriate to the G DOC Clinical Director and/or Head of Clinical Governance.
Management
Provide clinical line management to senior members of the pharmacy team, liaising with operational line managers and GP leads as appropriate.
Provision of appraisals for the clinical pharmacists in the locality.
Guide workforce planning and development within the locality pharmacy teams.
Ensure understanding and application of the organisational vision, mission and values.
