University Hospitals Dorset (UHD) is seeking a band 6 Specialist Speech and Language Therapist to join our forward-thinking Early Supported Discharge (ESD) Team — a leading service that transforms patient recovery by delivering specialist care at home.
We are able to create this new role due to recent investment in our service, and are excited to grow our team. Come and join us by the sea, at one of the biggest stroke service in the UK!
To provide assessment and intervention for people with communication disorders and dysphagia.
Assess, plan and deliver individualised programme of intervention evaluating the effectiveness for patients presenting with a range of conditions. Giving advice to patients and their carers when required.
Provide a high standard of interventions to patients with diverse presentations referred to the service.
Plan and carry out assessments of patients’ needs, taking into account information provided by carers and multi-disciplinary teams and develop and deliver an individual treatment programme.
To hold responsibility for own caseload working under the supervision of a Clinical Specialist Speech and Language Therapist.
Contribute to the discussions on service and policy development and implement in the workplace as directed.
Identify the current and potential future needs of the patient and be responsible for initiating appropriate referrals to a variety of internal and external agencies.
To assist the ESD Team Lead with data collection, SSNAP, audits and reports so that the service can be monitored and developed.
To assist with neurological education to therapy assistants, peers, students on placement, the wider team and external agencies.
Cross site working as required.
