A rewarding role has arisen for 2 substantive posts of an Enhance Care Clinical Specialist Nurse in West Hertfordshire Teaching Hospital (WHTH).
The Enhance Care Clinical Specialist Nurse will be part of the Enhance Care Team delivering timely quality care to patients within the trust.
We welcome applications from staff that are already on the NMC register with relevant experience in acute Mental Health inpatient settings.
The Enhance Care Clinical Specialist Nurse will be part of an MDT with partner agencies involved in the delivery of care to our patients.
You need to be a passionate role model in delivering compassionate care to our acutely unwell mental health patients presenting to the trust and keen to develop and build on your management and leadership skills.
You will support the Enhance Care Team & Mental Health Matron to lead the management and delivery of timely mental Health assessment, support & care, emphasising on relational security within trust, such as providing patient with choice, collaboration and transparency with a trauma informed care approach.
The post operates a shift rota system, including weekends and twilights. You will receive staff induction, in-house training, regular supervision and personal development reviews.
The appointed person will be expected to deputise for the Enhanced Care Team Lead, ensuring the effective coordination of enhanced care services. They will be responsible for risk assessing referred vulnerable patients in collaboration with ward teams to determine the necessity of assigning an Enhanced Care Support Worker. These referrals may be due to various factors, including challenging behaviour, dementia and delirium needs, a high risk of falls, and complex medical care support. Additionally, the role involves prioritising and allocating staff appropriately, overseeing workforce well-being, and supporting the Enhanced Care Team in daily operations. This includes workforce management tasks such as staff rostering, performance monitoring, and ensuring compliance to policies and protocols to maintain high-quality patient care.
Ensure that everyone is treated with dignity and patient’s needs are met in a timely manner. The focus of the role is to bridge the gap and supplement provision of mental support from the Mental Health Liaison Team (MHLT).
To complete trauma informed psychosocial mental health assessments and develop safe and effective collaborative plans of care with MHLT for patients within our emergency department and wards.
Provide compassionate and trauma informed care.
Responsible for the effective day-to-day management of clinical & operational issues for all mental health and neurodiversity patients in the Trust.
