NHS@Home is a model of care that delivers traditionally hospital-based care in people’s home, supported by joint working with system partners and the use of appropriate remote clinical monitoring tools.
Our Aims Are To
- Reduce unnecessary hospitals stays
- Promote confidence in the delivery of care in people’s home
- Promote confidence in a collaborative approach
- Promote shared decision making between patients and healthcare providers collectively
- Increase the positive approach to managing clinical risk of people managed in home settings across the system
Working closely with the Service Managers and Clinical and Operational leads, you will provide robust co-ordination across key areas of work within the service you support.
You will have overall responsibility for the collation and reporting of all quality assurance data and key performance indicators required.
You will provide project management support in service redesign and ongoing development of the service.
You will manage the NHS@Home administrative teams, providing line management, supervision, training and support; as well as supporting the Service Mangers diary and meeting organisation.
