The Role
As a Care Practitioner, you will play a vital role in delivering high-quality care and support to individuals accessing the Adult Social Care Service at Hungerford Resource Centre and you will work collaboratively with a team of dedicated professionals to ensure the needs and aspirations of service users are met.
Your Key Responsibilities Will Include
- Providing direct care and support to individuals, ensuring their physical, emotional, and social needs are addressed.
- Implementing care plans and monitoring progress, making adjustments as necessary to ensure optimal outcomes.
- Advising and supporting individuals and their families regarding available services, resources, and options.
- Collaborating with other professionals, such as healthcare providers and social workers, to ensure coordinated and holistic care.
The ideal candidate will possess the following skills and attributes:
- A strong background in care and support, with experience working in a similar role.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, families, and colleagues.
- Sound knowledge of relevant legislation, regulations, and best practices in care delivery.
- Proactive and self-motivated, with the ability to work independently as well as part of a team.
- Empathy, compassion, and a genuine desire to make a positive difference in the lives of others.
