Job Summary

This is an exciting opportunity to join our existing team of OHP clinical pharmacists working within practices across the Weoloey & Rubery Primary Care Network (W&R PCN).

The position of Primary Care Network Clinical Pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for Clinical Pharmacists to work with existing practice and PCN teams to deliver consistency and excellence in medicines optimisation across the member practices.

The successful candidates will be part of a dynamic and forward-thinking team of clinical pharmacists employed by Our Health Partnership, working across the practices within the PCN. Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacists in the PCN.

Please note whilst our Head Office is based in Cotteridge, this role will be required to work within any of the practices in the aforementioned PCN.

Main duties of the job

We are looking for pharmacists who will work as part of a multi-disciplinary team, developing and managing medicines optimisation services within our practices. Demonstrating improvements in value and patient outcomes and contributing to reducing health inequalities through the safe and appropriate use of medicines. This includes implementation of systematic processes for improving medicines optimisation across our PCNs and the wider OHP network. Work programmes will be prioritised according to the population needs of the PCN and in line with the PCN Directed Enhanced Service (DES).

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are currently a GP partnership of 30 practices with 39 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.

Job responsibilities

The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide clinical leadership on medicines optimisation and quality improvement (QI) and manage some aspects of the quality and outcomes framework and enhanced services at practice and PCN level.

The post holder will work with all sectors of pharmacy across the health system and the wider OHP organisation to improve population health. Demonstrating improvement in patient outcomes and working to reduce health inequalities across the PCN contributing to improving the efficiency of general practice and delivering a best in class service.

The post holder will be supported to develop their role in line with the OHP workforce strategy, this will include becoming and operating as a non-medical prescriber. This is a new role and will be reviewed at least annually, where the Job Description may be updated in line with the needs of the organisation.

Qualifications Essential

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • May hold or be working towards an independent prescribing qualification.

Experience Essential

  • Experience working as a clinical pharmacist.
  • Involves patients in decisions about prescribed medicines and supporting adherence as per NICE and local best practice guidelines.
