Are you passionate about making a positive impact on the lives of children and families? Starting Point, Derbyshire's leading front door service, is currently seeking dedicated Community Care Workers to join our dynamic team.
At Starting Point, we are driven by the goal of providing exceptional services to children and families across Derbyshire. As a Community Care Worker, you will play a crucial role in assessing requests for intervention and determining the appropriate support needed. The work environment is fast paced, requiring confident decision-making and effective teamwork. Building strong relationships with partner agencies and localities within Derbyshire is vital to ensure successful collaboration.
Starting Point is an office-based service and this position does not have direct face to face contact with service users or members of the public - therefore excellent telephone communication skills are required.
Our Community Care Workers are referred to as Referral Co-ordinators within our operating model.
There are 3 staff teams within Starting Point and Referral Co-ordinators on a rota basis support all areas of work. These teams are our Initial Response Team, Child Protection/Level 4 team and Child in Need/Emerging Needs level 3 team.
Referral Co-ordinators are responsible for information gathering and triaging work in line with Derbyshire’s Stronger Families Safer Children model and Derby and Derbyshire Safeguarding Children’s Board Threshold document.
Team managers, Practice Supervisors (Social work) and Social Workers initially screen incoming referrals and assign work across the service. Managers, Practice Supervisors (Social work) and Social Workers support staff with advice and discussion on thresholds.
You will be responsible for triaging the information provided in referral by gathering information from agencies and talking to Parental Responsibility holders about the child or young person. You will liaise with partner agencies - including providing advice, guidance and signposting to universal/community Services. In liaison with the Managers, PS and SW’s, threshold decisions will be made in terms of outcomes for the referral and further actions, whether this is no further action, or referral to the Early Help Team or Social Care.
There are local timescales set to ensure that work is completed in a timely manner.
You will be required to undertake further training and development.
Good IT skills and a working knowledge of Mosaic are essential.
