Description
The Diabetes Specialist Nurse will act as the clinical expert and professional resource to themulti-professional team, patients and carers within their Speciality Service. This role isprimarily community based.
The post holder will have a responsibility to help develop the specialist area of practice inpartnership with other members of the speciality team working collaboratively to developservices and improve the quality of care delivered.
The post holder will carry responsibility for a defined case load and offer specialist clinicaland service support and advice including setting standards of care, contributing to specificcare guidelines, and supporting the care of patients across the health economy. The postholder will also have an active role in the education of patients, carers and healthcareprofessionals in their area of expertise.
The post holder will contribute to achieving the objectives of the clinical servicegroup/speciality corporate objectives and work within the clinical governance framework. Thiswill be achieved through the core dimensions of the role, clinical practice, leadership, actingas the patient advocate and through audit and education.
