Generic Assessor – Nurse, Occupational Therapist, Physiotherapist
We are seeking a motivated, confident, compassionate and enthusiastic individual to join our excellent team.
If making a difference to our patient’s matters to you then this role may be exactly what you are looking for. It is an expectation that you will rotate through each of the 3 hub teams, developing your knowledge base, leadership, assessment and negotiation skills.
As a generic assessor you will need to be a forward thinking, autonomous practitioner, flexible, willing to develop interchangeable skills across professional boundaries, be knowledgeable on a range of conditions, their treatment and potential for deterioration. Enhanced training will be provided to assist with generic skills development.
We are inviting people with community or hospital experience to apply for this post with a professional background of nursing or therapy. This is an exciting time of forging links with our community colleagues, and embracing the current developments and projects.
- 1x Full time post = 37.5 hours per week
- 1x Part time post = 27 hrs per week
- Facilitate effective discharge practice by providing support and expert professional advice to all members of the multi-disciplinary team regarding patient pathways, minimising any identified risk, availability of services to promote rehabilitation and reduce the risk of readmission into the acute trust.
- In collaboration with colleagues from Community Services, Local Authority, Independent Care Providers and the Voluntary Sector provide a unified seamless service across all pathways.
- Identify patients within the Emergency Department and across the Assessment Units that are suitable for assessment thereby minimising the need for admission.
- Commence the assessment process using a holistic approach, drawing upon experience, specialist skills and evidence based principles to plan and implement a safe and timely discharge.
- Identify any barriers to discharge applying a resilient can do problem solving approach.
- Deputise for the team leader in their absence, ensuring that the needs of the service are met, allocate and deploy staff resource according to demand.
