Cardiff and Vale Health inclusion service (CAVHIS) is undergoing a large-scale expansion. The CAVHIS team aims to provide bespoke primary care to various health inclusion groups. The CAVHIS focus is on delivering holistic preventative care, limited primary care, bespoke health assessment, public health screening across community setting's and sites and outreach clinics, the in-reach service and our CRI hub.

Reporting to the Band 7 Team lead of CAVHIS the Paediatric- Health Inclusion Nurse Practitioner (HINP) will work across the CAVHIS service, as part of the expanding nursing team. Providing primary health care to improve the health outcomes of the most socially excluded patient groups in the Cardiff & Vale area. The HINP will take a positive, innovative and solution focused approach to providing timely care for people experiencing poor health outcomes who present with varying health complexities. As part of the CAVHIS nursing team, the HINP will need the ability to engage in transitional change and service improvement working closely with general medical colleagues, nurses from other specialities and tertiary agencies, to encompass the best achievable health outcomes. CAVHIS is currently implementing phase 1 of its developments and there is an opportunity for the right candidate to be a part of this exciting development in service.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Complete Initial holistic assessment of new Asylum seekers and Refugee's arriving in Cardiff and Vale.
  • Triage new arrivals when required
  • Immunisation
  • Public health screening- paediatric bloods and mantoux
  • To work as part of an MDT
  • Refer and signpost to relevant care/specialities/agencies where necessary
  • High level risk management, risk assessment and safeguarding awareness
  • To work as an autonomous practitioner when working with the CAVHIS team
  • Ability to work with people who have experienced trauma
  • Working with people where English is not their first language and using various means of interpretation
  • A non-judgemental attitude and compassionate acceptance of others is crucial to our service.
  • Working across health board and non-health board sites
