Description
The PBS Practitioner will hold a caseload of people we support, they will be involved in modelling and leading best practice, completing functional assessments, identifying and implementing strategies and appropriate support planning across the supports they work in. As a PBS Practitioner you will be a leader in your field and be able to train groups of support staff to embed Positive Behaviour Support across all the homes you work in. You will have knowledge and experience of working with people with learning disabilities, autism, behaviours that challenge and people who have experienced various levels of trauma in their lives. Your work will involve working with people with complex needs to ensure people are discharged from hospital settings or to avoid a hospital admission or readmission. Our focus is to enhance the lives of the people we support, improve the quality of life and ensuring we support people in the least restrictive way.

Responsibilities:

  • Respond to PBS Referrals in timely manner
  • Complete Functional Assessments and implement PBS Plans in collaboration with support teams, the person supported, and families as appropriate
  • To provide autism, Challenging Behaviour and PBS training to staff teams
  • To monitor implemented PBS Plans, to check on consistency of approaches
  • Coach/advise teams when inconsistency is evident
  • Respond to crisis situations in which a person is posing a risk to self and/or others and there is a risk of placement breakdown/hospital admission
  • Maintain data analysis related to behavioural support to evidence justification for strategies employed and behavioural trends over time
