Job Summary

We have an exciting opportunity for a newly qualified GP to join South Birmingham Alliance PCN.

We are looking for someone to work 6 sessions per week over 3 days - Monday, Tuesday and Thursday. You will be working across 2 of the practices within the PCN.

The successful candidate will be part of a dynamic and forward-thinking Multi-Disciplinary Team (MDT). Leadership will be provided by the PCN Clinical Director. You may have the opportunity to attend team meetings and there will be peer support available to access across the PCN. There will also be possible opportunities to attend and contribute to practice & PCN meetings. You will be provided with supervision from the PCN Clinical Director.

The role is offered on a fixed term contract basis, initially this will be up until 31/03/2025 (start date pending completion of recruitment checks).

Main duties of the job

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients across the Primary Care Network, dealing with a diverse range of health needs.

This will include, but is not limited to, surgery and telephone consultations, home visits, administration, complying with the practices contracts (GMS/PMS/enhanced services/DES).

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients..

We are currently a GP partnership of 30 practices with 39 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.

The partnership offers a shared administrative and management structure, cutting down the time doctors have to spend on admin. It opens up economies of scale to get best value from budgets. It has the resources to develop innovative services and effective partnerships with local hospitals and care services. And it can access new funding streams that are only available to large GP organisations.

Clinical Responsibilities

Main Duties & Responsibilities

  • In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion
  • Triage patients presenting to the surgery by telephone, online and in person
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education
  • Manage long-term conditions and patients with complex problems
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes and complying with QOF requirements
  • Delivering enhanced services

  • Refer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as required
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelines
  • Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
  • In general the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis
