Are you a dedicated and skilled Social Care Practitioner looking to join a dynamic team within the Adult Social Care Service at West Berkshire Council? We are seeking an enthusiastic individual to work alongside our Team Manager, reporting directly to them, to provide high-quality social care and support to our valued customers.

About The Role

As a Social Care Practitioner, you will play a crucial role in delivering exceptional social care services to individuals within the community; specifically working with young adults who are transitioning into Adult Social Care and who need care and support. Your responsibilities will include:

  • Working collaboratively with the Team Manager to ensure effective service delivery and meet the needs of our customers.
  • Conducting assessments and reviews of individuals' social care needs, identifying appropriate interventions and support.
  • Building positive relationships with customers, their families, and other professionals to promote well-being and independence.
  • Providing advice, guidance, and support to individuals, empowering them to make informed choices.
  • Ensuring accurate and timely documentation, including case notes, reports, and referrals.
  • Contributing to the continuous improvement of services through participation in team meetings and development initiatives.

The ideal candidate will possess the following skills and attributes:

  • A relevant social care qualification and experience working in a similar role.
  • Sound knowledge of social care legislation, regulations, and best practices.
  • Excellent assessment and intervention skills, with the ability to prioritise and manage a diverse caseload.
  • Strong communication and interpersonal skills, with the ability to build rapport with customers and collaborate effectively with colleagues and external agencies.
  • Demonstrated ability to work both independently and as part of a team, showing initiative and problem-solving skills.
  • A commitment to continuous professional development and staying up-to-date with industry developments.

Please note this role is fixed term for one year but may be extended to two years subject to funding
