An exciting opportunity has arisen to join the Discharge to Assess Community Team.
You will work as part of the Discharge to Assess Community Social Work Team to deliver on person centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention to support, provide guidance and signposting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014.
You will have demonstrable social work experience, the ability to work positively as part of a Multi-Disciplinary Team, in partnership with other stakeholders, together with an interest in and skills to work within the specific Discharge to Assess Multi-Disciplinary Team.
Working within the legislative framework, you will carry out social work functions, utilising relevant legislation to complete conversations and provide support and guidance, drawing on sound inter-personal and communication skills. You will require the ability to produce reports and the resilience to deliver under pressure within required timeframes.
You should be eligible to be registered with Social Work England. We are also excited to extend this opportunity to Social Work Apprentices and Students who have confirmation of their qualification (although is awaiting actual certificate) after completion of their studies this coming year.
