Community Learning Disability Team
The role of a Social Worker within The Community Learning Disability Team, is to undertake holistic strength-based assessments of adults with a diagnosed Learning Disability and to develop personalised support plans to meet the persons needs and outcomes. You will work collaboratively with a range of professionals (health care providers, education, housing etc) to coordinate care and ensure seamless service delivery.
Our teams work with service users, their families, and carers to enable them to be independent, fulfilled and to live within their communities.
We are looking for dedicated, hard working and a person centred Social Workers who are committed to developing high quality strength based packages of care and support , whom have experience of working with Adults and/or Children with disabilities, including multi-disciplinary working, knowledge of the Care Act, Deprivation of Liberty Safeguards, Mental Capacity Act, Special Education Needs and Disabilities (SEND) legislation, Personalisation and delivering a high quality service to our service users and carers.
We are also welcoming applications from newly qualified social workers / ASYE’s, who will be supported to complete the Assessed and Supported Year in Employment progression.

You must hold a
recognised Social Work Qualification and be registered with the SWE.
Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving license and it is expected that you will either use a council vehicle or your own car (exceptions may be made for disabled applicants).
