A new and exciting opportunity has become available, for an enthusiastic and forward-thinking Advanced Nurse Practitioner (ANP) to join our Neighbourhood Manchester Case Management team in the North of the city.

Manchester Case Management (MCM) supports people in our community, who are living with but are struggling to effectively manage, multiple long term conditions. The focus of MCM is manage escalating clinical need and risk, reduce reliance on unplanned health and/or social care, increase a person’s ability to anticipate and manage their own health needs and connect the individual to community support, via the neighbourhood Care Navigator.

The focus of the ANP role will be primarily to provide clinical oversight on the management of people on the MCM caseload, supporting the neighbourhood teams especially across the interface between neighbourhood and urgent community care.

As the lead clinician within MCM, the post holder will be expected to:

actively promote a person centred, strengths based approach to care and support

support a culture of continuous improvement

be responsible for managing escalating clinical need and associated risks

The post holder will be an autonomous practitioner, who will be expected to take an active role in responding to and managing the increasing clinical risk, of patients who are on the Co-ordinated Care Pathway (CCP).

This role will provide comprehensive healthcare, sensitive to the needs of the patient population. This will include advanced clinical assessments, history taking, diagnosis, care planning and delivery, prescribing and evaluation of the team’s intervention.

Working within a multidisciplinary team, as well as across the primary, secondary and social care interfaces in Manchester, (for both physical and mental health), the post holder will be responsible for co-ordinating the delivery of health care, which is person-centred and meets the escalating needs and associated risks, of people on the CCP.

The post holder will lead in developing, delivering and evaluating high quality patient centred interventions, which patients are satisfied with and which align to the delivery of Manchester Local Care Organisation’s objectives.

The post holder will provide clinical support and advice to case managers within MCM and act as a resource for other clinicians working within the Integrated Neighbourhood Teams (eg: /District Nursing).

The post holder will be expected to undertake a day a week within urgent care services to maintain clinical skills and support integration between neighbourhoods and urgent care.
