An exciting opportunity has arisen for a dynamic and experienced Advanced Community Nurse Practitioner to join the Enfield District Nursing service.
The Advanced Community Nurse Practitioner carries the continuing responsibility for the health care management of a particular geographical area as aligned with Primary Care Network (PCN) and with the responsibility for clinical leadership to optimize quality and standard care service delivery.
The post holder strives to continually improve care pathways for patients, their carers, and families when utilising district nursing services in conjunction with other statutory and voluntary services.
The Advance Community Nurse Practitioner is a specialist role who provides holistic care and promotes independent living. The post holder will be a senior nurse with a Specialist Practitioner District Nursing Qualification or equivalent and has significant Community Nursing experience to provide expert clinical advice to support the District Nursing Team.

- To provide highly developed clinical leadership for all patients on the District Nursing caseload through advanced level autonomous practice including taking managed risk.
- Highly provides specialist clinical leadership for patients on the caseload and works collaboratively with all professionals, carers, and relatives to gain a deep understanding of all aspects of the patient’s physical, emotional, and social situation.
- This includes physical examination and detailed history taking, diagnosis, prescribing, planning and evaluation of care
- Highly provides specialist clinical leadership for the development and maintenance of personalised care plans for patients on the community nursing caseload, based on the full assessment of medical, nursing and care needs. This includes preventative measures and anticipation of future needs..
- Order investigations and tests and interpret and act on results, using clinical guidelines and policies to support clinical decision-making.
- Maintain competence in prescribing according to the level of prescribing qualification i.e., Nurse Prescriber or Independent Prescriber and participate in regular continuing professional development in relation to the role.
