Plymouth City Council Independence at Home Management Team are pleased to advise, that although we are always sad to see one of our colleagues retire after 18 years of dedicated service, a rare opportunity has become available for a permanent post as a Reablement Care Coordinator [Grade E] FTE SCP 15 -20, attracting a yearly incremental rise, salary starting point £29,093.00 to £31,586.00, 2024/25.

Salary is annualised, representing a 30-hour contract. The successful candidate would be contracted on rotation over a three-week period with an established team of coordinators.

  • Week 1 Monday, Friday 16:30-23:00, and Saturday and Sunday 14:45-23:00
  • Week 2 Tuesday, 16:30-23:00, Saturday and Sunday 06:45-14:45
  • Week 3 Wednesday and Thursday 16.30-23.00

The role attracts annualised bank hours and has element of home working in the evenings.

We are looking for a dynamic, sensitive, and enthusiastic individual to join the established professional care team. This is a real opportunity for people who want to work as part of a wider team to support those who have a variety of needs to retain and/or optimise their independence at home and contribute to their wellbeing.

F ull training and support is provided but you will already have some really important skills and understanding that you will be able to bring to this rewarding role

These Include

Effective communication skills, verbal and written to build rapport with individuals and record and report on aspects of their health and well-being to the care coordination team.

Time management and organisational skills.

IT and scheduling software knowledge

A reasonable understanding of the need for dignity, respect and sensitivity when working with vulnerable and older people

The ability to motivate and work with service users some whose behaviour may be challenging, disengaged or who are terminally ill.

Ability to lead and listen to concerns and deal with them appropriately

It is essential that you have the following: -

GCSE (A-C) or equivalent in English and Maths

Health and Social Care Diploma Level 2

Full Driving Licence and access to a vehicle for work purposes

A commitment to continuous professional development in accordance with National Care Quality Standards and Plymouth City Council’s training plan.
