The Dorset ME/Chronic Fatigue Syndrome Service has an exciting opportunity for an Assessor/Case Managers (ACM). The post is based at Wareham Hospital BH20 4QQ
We offer specialist assessment, intervention and review to people in Dorset diagnosed with ME/CFS. You will join a skilled multidisciplinary team delivering high-quality, NICE-concordant care. The role includes: carrying out assessments, holding a caseload, providing evidence-based interventions, carrying out reviews and working closely with colleagues to support recovery.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

To be responsible for managing a caseload of people who have been referred into the ME/Chronic Fatigue Syndrome Service.
ACM will have responsibility for assessing individuals referred into the service and supporting diagnosed patients throughout their onward intervention journey
ACM will conducting initial assessment discussing the person’s signs and symptoms of ME/CFS and the impacts on function that have developed. This will be completed and recorded on the clinical records system
Accurately record outcome of assessments and all interventions on the clinical record
Through the Case Management function offer the person support and co-ordination throughout their intervention from the service
