This is an excellent and rare opportunity for a highly motivated and enthusiastic nurses to join the specialist palliative care Team at East Lancashire Hospitals Trust
You will support the delivery of a specialist palliative care nursing, seven-day service, across the hospital and community settings.
Generalist palliative care experience is needed for this role, and must be demonstrated in your application form.
Please be advised you may be required to work across both the hospital and community settings. subject to your development to and to meet service demand.
Staff will be expected to work flexibly over seven days, including weekend and Bank holidays.
The post holder will contribute to the provision of a co-ordinated and comprehensive specialist palliative care service for patients, families and carers across ELHT.
Have responsibility for a case load of patients with Specialist Palliative Care Nursing needs.
Assess patients, formulate management plans and implement care by providing specialist nursing advice on complex pain and symptom management and maintain associated records.
Act as a specialist resource for advice on complex pain and symptom management to health care professionals across ELHT.
This is a very rewarding and emotionally demanding role caring for people and their families who require Specialist palliative Care services. You will be working in a team consisting of Clinical Nurse Specialists, the Lead Nurse and Assistant Director of Nursing, Consultants in Palliative Medicine, an Advanced Clinical Practitioner and the administrative team, and working closely with the End of life and Bereavement team.
You will play an active role, together with the existing members of the Specialist Palliative Care Team, in advancing high quality specialist palliative care services and promoting excellence in palliative and end-of-life care across ELHT, and in return you will be welcomed into a friendly and supportive team, who are mindful of the demands of this role, and who recognise that peer support is pivotal to staff health and wellbeing.
