Job Overview

The post holder will be practising under supervision as a trainee practitioner within cardiac rehabilitation. This will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout cardiac rehabilitation and related services. The post holder will aim to work towards the Advanced Nurse Practitioner Qualification. The training programme involves a combination of workplace-based learning of clinical skills, attitudes and knowledge and university based academic modules. The academic rigor of this level requires dedicated study time.

The training programme should be completed, subject to satisfactory assessments. The trainee will then progress to being a band 7 Advanced Cardiology Nurse Practitioner.

Main duties of the job

An exciting opportunity has arisen to join our experienced team of specialist nurses in the delivery of Cardiac Rehabilitation. We are seeking a nurse who possess a nursing background in Cardiology and who has completed a Masters in advanced clinical practice, is already working towards completion or will commit to complete in the future. The purpose of the role is to provide specialist Cardiac Rehabilitation knowledge, expertise and skills for patients and their families that promotes recovery and self-management. This requires you to work both autonomously and as part of a multi-disciplinary team to ensure a safe and effective pathway for cardiac patients.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Detailed Job Description And Main Responsibilities

The post holder will support the patient through their Cardiac Rehabilitation pathway by remote and face to face consultation. You will work as part of the Cardiac Rehabilitation Team and the wider Cardiology MDT. Whilst your base will be in Ysbyty Glan Clwyd you will be expected to support clinics in the community.

Knowledge

Essential Criteria

  • Advanced Nurse Practitioner MSC or willingness to complete/undertake
  • ILS or willingness to undertake
  • Familiarity with practical application of cardiac rehab e.g. BACPR standards

Desirable criteria

  • Post-grad level module in cardiac care
  • Relevant course or training in cardiovascular care
  • Understanding of National Audit of Cardiac Rehab

Essential

  • Excellent time and resource management

  • Recognised Qualification in leadership/change management.
