Sefton Council is seeking to recruit a Care Arranger Coordinator to work as part of an existing Care Arranger / Brokerage Team, but with a focus on the securing of Health care packages and care home placements.

The Role Will Have a Key Focus On

  • Supervising staff within the Integrated Care Arranger team across Adult Social Care and Integrated Care Board (Health).
  • Ensuring that commissioning frameworks are followed when securing care services for individuals and that value for money is secured.
  • Supporting social care and health colleagues to ensure that brokerage information is clear and that processes and frameworks relating to commissioning support services are followed.
  • Liaising with care providers, individuals who receive services, their families and advocates and Adult Social Care/Health colleagues regarding care services.
  • Working closely with Commissioning colleagues to identify appropriate resources to best meet needs, identify gaps in the market and ensure new and existing services meet peoples needs
  • Putting people at the heart of what we do and be responsive.
  • Maintaining a customer focus with a relentless pursuit of excellence, developing services that improve outcomes for the communities of Sefton.
