Are you a highly organised, people focused professional with a passion for making a difference? Candlelight live-in care is looking for a motivated Care Coordinator to join our team.
We support vulnerable people to continue to live in their own homes, this is a rewarding role.
As our Care Coordinator, you will be the key link between our clients and care workers, ensuring a smooth service delivery.
Role as Care Coordinator
- Responsible for allocation of Clients and Carers and the communication of the allocations through the distribution of booking confirmations and rotas.
- Be the point of contact for Clients and Carers concerning their Rotas.
- Maintain accurate and up to date information on Access.
- Develop good relationships with carers, liaise with clients and Partners/Stakeholders/Commissioners in a professional manner, and deliver excellent customer service.
- Assist the Office team with the onboarding of new Clients and Carers onto the system and the digital file counterparts.
- Provide support with telephone calls and emails to help ensure the service runs smoothly and efficiently.
- Work as part of the Candlelight Live-In Care team, which means striving towards the same goal and supporting others whilst allowing others to support you.
- Promote, represent and uphold the Candlelight values of independence, choice, dignity, respect, and desire to keep Clients at home for as long as possible.
What we are looking for:
- Strong personal impact and relationship building skills, with the capability to engage with and influence Carers and Clients - be a people person.
- An excellent listener, able to tune your ear to those who may have difficulty communicating or whose first language is not English.
- To be able to effectively communicate across and relay both verbally and in written format.
- To be resilient, compassionate, professional and a real team player.
- You will be able to adapt to a fast-paced changing environment and know that nothing is impossible.
- Competent IT skills are essential as is an eye for detail and a desire to deliver a quality service.
- Take responsibility for time management, in prioritisation of essential jobs, ongoing projects and personal work life balance.
- Experience in care.
