Care Coordinator

Delta Care Chorley is currently seeking a dedicated and experienced Care Co-ordinator to join our dynamic team. You will play a key role in supporting the Registered Manager and the team, ensuring the smooth coordination of our Domiciliary Care Workers across the Chorley area. About the Role:

We’re looking for an exceptional individual who is passionate about delivering high-quality care and has a proactive, solutions-focused approach. You will be responsible for the effective rostering of staff and ensuring the delivery of person-centred care to a wide range of clients.

Requirements:

• Previous experience in Health and Social Care
• NVQ Level 2/3 in Care (or above)
• Full UK driving licence (essential)
• Strong understanding of the principles of high-quality care
• Excellent communication and relationship-building skills
• Ability to plan, problem-solve, and remain calm under pressure
• A warm, empathetic approach with a commitment to person-centred care

Key Responsibilities:

• Coordinating care staff schedules and rotas
• Liaising with clients and care workers to ensure smooth service delivery
• Supporting the Registered Manager in day-to-day operations
• Responding to emergencies and last-minute scheduling changes
• Ensuring compliance with quality and regulatory standards
• On-call duties (1 in 4 weekends & 1 night every week )
