Job Overview
To work as part of the Children’s Community Nursing Team as an experienced band 5 nurse, providing nursing care primarily within a child’s home, ensuring the delivery of evidenced based care to children and their families or carers. To apply for this post you must have a NMC Child Nursing registration. The application must have a full driving licence. and access to a car for business use.
Main duties of the job
Clinical
- to participate in and critically evaluate the assessment, planning and provision of skilled nursing care in negotiation with the child and family, in conjunction with the CCN team leader
- to provide skilled, effective, evidenced based nursing care for children within their own homes including technology dependent such as tracheostomy care and central venous access.
- To teach parents and carers skills to enable them to look after their child at home
- To provide information and support to facilitate the child and family’s own choices with regards to nursing care
- to promote Health Education as an integrated aspect of care delivery

Management
- to facilitate and encourage a learning environment and be actively involved in meeting the educational needs of learners
- to be responsible for the care of equipment issued to a home and to report any defect or loss
- to participate in Child Protection procedures.
- to establish and maintain good relationships with the members of the multi-disciplinary team, for example, Acute services, Tertiary Centres, Social Services and other agencies
- to actively participate in the orientation and development of new staff to the community
- to participate in developing the CCN team
Administration
- to maintain accurate records and statistical returns as required by Provide
- to participate in team and multi-agency meetings / conferences.
- to take appropriate action in regard to any accident or incident to a child, family member, staff member or visitor
- to be aware of Provide policies and act accordingly
- to maintain and update child’s care plan in partnership with family and key professionals involved in that child’s care.

- to be aware of and act in accordance with the Nursing and Midwifery Council Code of Professional Conduct
- to participate in clinical and safeguarding supervision in accordance with Provide policy
- to participate in reflective self- appraisal and Personal Development Planning review

- Communicates issues relating to safeguarding children with the child, families and MDT.
- The post holder will be responsible for contemporaneous record keeping.
There may be a requirement to work outside of normal working hours to facilitate end-of-life care at home with the expectation to support with on call duties which includes overnight and weekend cover.
