Evelina London is at the heart of a network of children’s health services in Lambeth and Southwark and across South-East England. We provide a comprehensive range of children’s health services, so that we are able to look after patients from antenatal diagnosis through childhood, into adolescence and on into adult services.
The post holder will be responsible for facilitating early discharge, reduce length of stay and prevent hospital admissions. We are seeking enthusiastic experienced paediatric nurses, who have a passion for acute care in the community, to work as part of the existing team to support an exciting new pilot in digital health technology for remote monitoring on our virtual ward.
You will work as an autonomous practitioner assessing patients’ needs and using evidenced based clinical skills alongside digital technology to care for patients in their own home. You will also be responsible for caseload management, handover of care and service developments providing specialist input when necessary.

Our current working hours are 7 days a week, 0800-2200 for hospital@home on a shift basis.

The successful candidate will join the established Children’s at Home team within our Children’s Community Nursing Service, in the delivery of quality care for children and young people. This is an exciting opportunity to be part of a new development working with patients in their own home with high acuity needs. This will be integral in enabling patients to achieve optimum function and receive appropriate treatment within their own home that will prevent unnecessary admission to hospital.
You will be supported by the senior clinical team including ACPs, trust doctors/consultants, respiratory physiotherapists, and senior pharmacists. There will be daily board round meetings held to discuss the patients cared for on the Virtual Wards and a weekly MDT meeting.
We will provide the successful candidate with the relevant in-house training and further education supported by our education department. You will be enrolled on a community development pathway that will include Evelina London Foundations in Children’s Community Care (EL FiCC) and Evelina London Paediatric Advanced Clinical Assessment (El Paca) that will mirror an enhanced skills pathway.
Previous experience of working in a community setting and/or working autonomously with acute nursing skills is essential for this post.
