This is an exciting new role within the Care of the Elderly. There is an opportunity of both permanent full time and part time up to 30 hours, for the role of the Clinical Coordinator.
The Pinderfields Acute Assessment care of the elderly Unit, G41 is a busy 44 bedded, fast paced, busy assessment unit. The unit takes take direct admissions from the Mid Yorkshire trust 3 Accident and Emergency departments as well as GP referrals. We are also recruitment for G42 which is a busy 45 bedded Elderly Medicine and Neck of Femur unit and G43 which is a busy 44 bedded Elderly Medicine Unit.
We are looking for an enthusiastic and passionate individual to join and become a clinical coordinator who is eager to develop their own knowledge and skills.
As part of your duties, you will be required to support the nurse in charge in delivering, managing patient flow through the department safely, coordinating the safe admission, transfer, and discharge of patients.
To be considered for this post, you must have clinical experience and preferrable experience of managing patient flow but is not essential for the right candidate.
Ideal candidates need to demonstrate good organisational skills, is a logical thinking person, can prioritise their own workload and help support the nurse in charge in relation to patient flow. Your role will involve close working with the patients, patient flow teams and emergency departments to ensure flow through the units. You will be working closely with members of the multi-disciplinary team and good communications skills are vital in the planning and delivery of care.
Furthermore, you will be required to undertake clinical skills such as cannulation & venepuncture, as well as provide support to the wider nursing and clinical team. These skills are desirable but training and support to achieve these skills will be provided.
Main duties
To carry out administration duties in a manner that supports departmental targets and ensures compliance with standard operating procedures.
Key Requirements
Responsible for the daily update of the Patients Administrative System. Provide an accurate account of admissions, discharges, transfers and deaths.
Maintain accurate records of patient flow through the Unit.
Assist the ward coordinating, supporting with the flow of patients, including dealing with telephone enquiries contacting patients families etc whilst maintaining confidentiality at all times.
Provide cover where needed for absent colleagues, under the supervision of the Senior Sister/Charge Nurse.
Update knowledge of internal software programs such as PAS, Symphony and PPM+.
Process referrals in a timely and sensitive manner, informing clinical staff as required.
Undertake extended clinical skills such as venepuncture, cannulation, catheterisation and ECG recording.
Support the Trust’s Infection Control and Prevention agenda at all times.
Ability to provide essential patient care to patients when required to support both registered nurses and Healthcare Assistants clinically, in line with the band 6 AAU coordinator role.
