- To support the effective use and management of the inpatient resource, ensuring that capacity is maintained in the local inpatient wards to enable those patients with highest need to be supported close to home.
- To become a member of the leadership teams in the adult acute wards, building relationships based on trust, openness and transparency.
- To support the development of home treatment care plans that meet the immediate needs of patients and their carers/significant others at the point of early discharge from the inpatient wards using the entire resource that the CRHTT has on offer.
