Description
We have an exceptional career development opportunity for an innovative and inspirational leader to join our Acute Frailty Team as a Clinical Nurse specialist for older people.
The CNS for older people and Frailty will take a senior role within the Care of Older People (COOP) team. They will provide a role which is 80% clinical, 20% quality improvement, service development and management.
The post holder will be responsible for co-ordinating the flow of frail older people through the Whittington hospital and ensure access to appropriate community services. The CNS will provide a clear focus for clinical leadership and be responsible for ensuring the provision of a high standard of holistic, patient centred care for frail older people.
