About The Role
We are seeking a motivated and compassionate Band 6 Clinical Nurse Specialist to join our paediatric diabetes team. This is an exciting opportunity to provide expert nursing care and support to young people aged 16-19 years living with diabetes, working collaboratively with a multidisciplinary team to improve patient outcomes and promote holistic care. We welcome applications from experienced paediatric nurses.
The post holder will deliver specialist diabetes nursing care, providing expert clinical support and education to children and adolescents up to 19 years old, as well as their families and carers, within the Wakefield district. Care will be delivered in line with agreed policies and protocols, underpinned by the latest research and evidence-based practice, and aligned with both local and national standards. The successful candidate will support 16-19 year olds through their transition to adult services.
The post holder will play a key role in empowering children and young people with diabetes to actively engage in their own care, fostering collaborative partnerships with healthcare professionals. They will support patients in setting realistic and achievable personal goals to enhance self-management and wellbeing. This role involves working across the district, providing cover and support at Pontefract General Infirmary and Pinderfields General Hospital, as well as delivering care within the designated Paediatric Unit.
Main duties
- Will have continuous delegated responsibility for a caseload of patients up to the age of 19 with type 1, 2 or other forms of diabetes. They will provide an effective and patient focused service which is considerate on needs of users and providers.
- Organises, prioritises and evaluates workload based on accepted referral criteria and discharge criteria in line with operational protocols.
- Will provide clinical, social and psychological support to their patients using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health.
- Will undertake specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills.
- To provide specialist advice (automated insulin delivery, multiple daily injections, variable doses and insulin adjustments, interpretation of continuous blood glucose monitoring, carbohydrate counting), supervision and support to their defined caseload and peers, in a variety of settings (hospital, home, school, via telephone or face to face).
- Responsible for providing evidence based practice.
- Will need to participate and liaise with other professionals regarding safeguarding children and young people including referrals to social care and attendance of child in need meetings and child protection conferences
- Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
- Support young people through the established transition service into young adult care
- Will have responsibility for supporting the Band 7 CNS / team leader in leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement.
- Will ensure cost effective use of all resources and appropriate governance management.
- Promote programmes of health education and health promotion relevant to service.
- Work within national policy frameworks relevant to the service and trust local guidance.
- Review , interpret and act upon diagnostic investigations and point of care testing which may effect, change to individual care plans, liaising with appropriate medical staff where necessary.
- Deal effectively and professionally with patient / service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints and escalate when unable to resolve
- To maintain accurate appropriate and concise patient records within agreed databases and electronic systems.
- Contribute to the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs.
- Act as a resource for specialist knowledge for clinical staff on wards/ departments as well as other disciplines in primary, tertiary care, education, and social services.
- Participation in the diabetes on-call rota to be expected when competent
- Ensure effective multi- disciplinary communication.
- Support the management function of the department by provision of reports and information as required.
- Provide mentorship and preceptorship as required to junior staff in the team or students on placement.
- Ensure you work within an agreed divisional work plan.
- Work as an independent non- medical prescriber within the area of practice for the CNS role taking responsibility for prescribing a range of drugs within area of competence in accordance with MYHT medicines management policy and BNF
- Complete annual declaration of intention to practice non medical prescribing in accordance with MYHT non- medical prescribing governance framework
Essential Person Specification Requirements
- Registered Children’s Nurse/ Registered Nurse
- First level (Honours) degree or willing to

- Post registration qualification in relevant subject
- Evidence of continued professional development
- Recognised teaching qualification

- Post registration experience of Band 5 or above in a relevant area
