This post is fixed term/secondment 12 months to meet the needs of the service.If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

An exciting opportunity has arisen for motivated and enthusiastic individual to join our expanding pharmacy team, to lead new mental health clinical pharmacy services across our community teams as part of our mental health services. This role will lead a pharmacy team within BCUHB West to develop and improve the community mental health pharmacy teams across adult and older adult mental health services. This role is a great opportunity to individualise and develop the service. Applicants should be patient focused and proactive in developing and using your clinical skills and knowledge to improve care through the delivery of efficient medicines optimisation to reduce inequalities and improve care of mental health patients in the community. Applicants need to be able to work well both within the pharmacy and wider community mental health teams but also be able to practice autonomously, undertaking individual patient assessments and consultations as required. Experience of working within community mental health teams is desirable but not essential and applicants should be or be willing to work towards independent prescriber within a mental health speciality. The base will be split between the main Ysbyty Gwynedd site and other community mental health bases in BCUHB West.

Main duties of the job

Lead a safe and effective specialist clinical pharmacy service in allocated area in community mental health settings

Undertake professional and clinical pharmacist’s duties

Provides expert pharmaceutical advice and support within both primary and secondary care services as required

Develop, encourage and promote evidence-based, cost effective use of medicines.

To be involved in the training of diploma, pre-registration and other pharmacists in area of speciality

Assist in providing clinical training and supervision of pharmacy staff and other healthcare professionals

To be involved in the development and delivery of practice research, service and quality improvement

Develop policies, protocols and guidelines within allocated area. Work in partnership with service users, other health and social care colleagues, to achieve the optimum health outcome for service users

Work within the scope of practice

Input into multidisciplinary team meetings within psychiatry

Provide specialist advice and support to health care professionals in community mental health teams and primary care to improve medicines optimisation in mental heath

Lead incident reviews and other clinical governance issues related to psychopharmacology across community mental health as directed by lead pharmacist
