The colorectal stoma care Nurse Specialist (CNS) will act as the clinical expert and professional resource to the multi-professional team, patients and carers within their specialist service. The post holder will take overall responsibility to lead, manage and develop this specialist area of practice. This will include the management of a caseload of patients, as appropriate. The post holder will have an important role with patients, their families and other clinicians raising awareness of signs and symptoms of bowel disorders and disease and the possible diagnostic and treatment options. His or her own working practice should be developed to a level whereby the nurse specialist can directly or indirectly influence all aspects of care and management of patients with colorectal disorders in particular those who have or due to have stoma formation. This role will necessitate carrying out colorectal cancer follow up.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
- Demonstrate clinical knowledge and interpersonal skills in caring for patients with colorectal cancer and colorectal diseases with a particular emphasis on stoma care.
- Participate in establishing and running colorectal nurse led virtual colorectal cancer follow up clinics, pre-admission clinics and attend joint new patient and follow up clinics.
- During pre admission clinic, where appropriate offer Health Needs Assessment and referral to supportive cancer services in addition to carrying out pre operative stoma counselling and siting
- Demonstrate clinical decision-making skills in referring patients for further tests or discharge to GP.
- Work towards developing advanced practice functions including clinical history taking and physical examinations skills.
- Co-ordinate the patient care pathway
- Provide continuity of patient care on a daily basis by following the patient’s journey from pre admission/admission, discharge and follow up/continuous support and encourage ERAS principles during the patients hospital stay.
