We are looking for a highly motivated individual to join the Learning Disability Services for Mersey Care NHS Foundation Trust - you will be based with the Halton Community Learning Disability Team at the Bridges Learning Centre in Widnes.
As part of your role you will be supporting individuals with a diagnosis of learning disability with mental and or physical health issues, supporting them in many aspects of their life and potentially a number of different venues across the community.
In return you will join a supportive and enthusiastic team and receive regular supervision, training to develop skills across a number of pathways.
The post-holder will be responsible for the assessment of health care needs and the development and evaluation of Care Plans for individuals referred to the Service.
The post-holder will have responsibility for prioritising and managing a supervised caseload delegated by the Team Manager and be responsible and accountable both professionally and legally for all aspects of
clinical and non-clinical activity in the service area, ensuring a high standard of clinical care is delivered to the people with learning disabilities under their management.
The post-holder will be required to work in diverse environments away from the main area and not directly supervised but will be under the guidance of a Senior Nurse.
