• To manage own caseload of Children with a Learning Disability, which will be identified by Senior Community Nurse for Children with a Learning Disability.
• To plan care for Children with a learning disability and be a support to them and their families according to the care pathways.
• To be a competent, autonomous community practitioner, with support from senior members of the Learning Disability Team.
• To work as a member of Children and Young Peoples service to promote a seamless provision for children.
• To provide care in partnership with parents/carers /children using interventions appropriate to the assessed need as per the C NCLD Standard Operating Procedures .
• To evaluate the outcome of interventions using a process of regular review in collaboration with the child, family, C NCLD and other professionals involved.
• To advise on the promotion of health and the prevention of ill health.
• Participate in Safeguarding Children processes as required , sharing information as necessary and completing reports within timescales.
