- To manage own caseload of Children with a Learning Disability, which will be identified by Senior Community Nurse for Children with a Learning Disability.
- To plan care for Children with a learning disability and be a support to them and their families according to the care pathways.
- To be a competent, autonomous community practitioner, with support from senior members of the Learning Disability Team.
- To work as a member of Children and Young Peoples service to promote a seamless provision for children.
- To provide care in partnership with parents/carers/children using interventions appropriate to the assessed need as per the CNCLD Standard Operating Procedures.
- To evaluate the outcome of interventions using a process of regular review in collaboration with the child, family, CNCLD and other professionals involved.
- To advise on the promotion of health and the prevention of ill health.
- Participate in Safeguarding Children processes as required, sharing information as necessary and completing reports within timescales.
