What is the job role?

The Adaptations team is part of the Therapy and Enablement Service. The Adaptations team are responsible for administering Disabled Facilities Grant (DFG) applications, the outcome of which is to enable clients to remain as independent as possible in their own homes.

Typical adaptations include altering bathrooms to create level-access showers; creating ramps; widening doorways; and sometimes large extensions to properties.

As a Case Worker within the team you will be supporting clients to apply for a DFG, and case managing their application from start to finish, being their main point of contact throughout the process. You will be responsible for completing financial assessments and ensuring that all DFG applications are complaint.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?

We are looking for a dynamic individual with excellent organisational and communication skills. You will be methodical, yet have the ability to be person centred and adapt your approach depending on the client you are working with. You will provide case management support to the technical officers whilst ensuring that the clients journey is both supportive and timely. You will have the ability to prioritise and manage your time effectively within the busy department. You will have good knowledge of DFG legislation.
