We are recruiting a Dietetic Advanced Clinical Practitioner to support people with diabetes and the existing team. This role will be based at the Leicester General Hospital and will cover Leicester, Leicestershire and Rutland.
Come and join us in this new and exciting venture, blending medical nursing and dietetic services together to support the diabetes virtual ward, bridging the gap between hospital and primary care. We believe in delivering safe, effective and personalised health care at the patient’s own home, and you will be working with the diabetes specialist nurses who are currently supporting people with diabetes in their own home. The role will be predominately based at the Leicester General Hospital but there may be some cross site working.
The Diabetes virtual ward aspires to help people to stay well and at home by facilitating self and supported monitoring of their condition. The diabetes virtual ward has been running for the last 2 years with great success, we are now expanding the service to include direct referrals from East Midlands Ambulance Service and we hope to extend to be able to provide a Diabetes SDEC service.
Currently we have funding until the end of March 26 but the outcome results for this ward have been extremely positive so the indications are that this post will continue to be funded post this fixed term period.
You will be providing a comprehensive dietetic assessment, treatment and management service as part of the integrated MDT in diabetes. The client caseload is varied and covers all aspects of diet and nutrition. Consultations will be a blended approach of both in person, and remote monitoring – there will be some home visits involved. You will work closely with the Advanced Nurse Practitioner Lead in developing and evaluating the Service.
