An exciting opportunity has arisen to join our dynamic Geriatric Emergency Medicine Service (GEMS) at North Middlesex University Hospital as an Advanced Clinical Practitioner (ACP). You will work autonomously and collaboratively across the Emergency Department (ED), Same Day Acute Frailty (SDAF) pathway, and acute frailty ward (Amber) to deliver high-quality, patient-centred care to older people living with frailty.
You will be a clinical expert, responsible for comprehensive assessments, diagnostics, treatment, and discharge planning. As a senior member of the frailty team, you will also lead service development, quality improvement, and education initiatives across the front door and community interface.
This is a FTC post for 9months covering secondment.
Main Duties And Responsibilities
- Provide expert frailty assessment and management across ED, SDAF, GEMS, hot clinics, and Amber ward.
- Work autonomously in clinical decision-making, including prescribing and diagnostic interpretation (where qualified).
- Develop and implement care plans in collaboration with patients, families, and the multidisciplinary team (MDT).
- Lead discharge planning and admission avoidance through liaison with community and social care partners.
- Line manage, support, and supervise frailty team staff, including junior ACPs and practitioners.
- Facilitate frailty education and training across acute and community settings.
- Lead quality improvement, audits, and innovation projects in line with the Trust frailty strategy.
- Participate in service evaluation, workforce development, and governance activity.
- Act as a professional role model and promote evidence-based, person-centred care
