Job Overview

This post is Fixed term/Secondment until 05/10/2026 to cover maternity leave.

An exciting opportunity has arisen for an enthusiastic Band 4 Therapy Assistant to join the discharge to recover and assess (D2RA) and Assessment and Discharge Team (ADT) service in the Central area of BCUHB. This service aims to facilitate early discharge from hospital and prevent admission to hospital. You will be working with patients with a wide variety of diagnoses providing support to the Occupational Therapists and Physiotherapists to provide assessment and intervention in their home environment.

The team comprises of Occupational Therapists, Physiotherapists and generic therapy assistants and you will be working closely with both hospital and community multidisciplinary teams.

You will have excellent communication and time management skills and be able to work flexibly to meet the needs of this developing integrated service.

Main duties of the job

Significant current experience of working within acute/community services is essential. You will play a supportive role in the development of this service whilst also ensuring person centred care is delivered in a timely manner, to patients in their own homes. Recent and relevant experience in a therapy healthcare setting is desirable.

You will deliver routine physiotherapeutic treatment programmes and delegated occupational therapy intervention plans independently and progress treatments within guidelines and protocols. You will be accountable for these tasks.

The successful candidate will be responsible for some of the day to day administrative duties within the department. Proficient computer skills are also essential as documentation is electronic.

The team are based in Glan Clwyd Hospital and the successful applicants will provide support across Conwy and Denbighshire.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
