Job Overview
The HCC Surveillance Digital Navigator will be a key member of the liver cancer surveillance team, responsible for supporting the operational delivery of a safe, effective, and equitable digital surveillance pathway for patients at high risk of hepatocellular carcinoma (HCC). This includes managing a secure digital database, coordinating ultrasound appointments, tracking compliance, and ensuring no patient is lost to follow-up.
Working closely with hepatology clinicians, radiology, the Early Detection Liver Cancer Nurse, administrative staff, Cancer Services, and the NBT/UHBW Digital Transformation teams, the post holder will provide continuity, coordination, and a single point of contact for over 2,000 patients undergoing six-monthly HCC surveillance. This role will also support equitable access to care across the hospital group, in alignment with NICE and NHSE national standards.
Main duties of the job
- Transfer and maintain records of all eligible patients within the HCC digital tracker system (Careflow or equivalent).
- Monitor patients’ surveillance schedules and ensure timely coordination of six-monthly ultrasound scans and AFP blood tests.
- Identify missed appointments or delays and escalate for appropriate action.
- Liaise with radiology, hepatology, and admin teams to book follow-up or reschedule scans.
- Provide non-clinical information and reassurance to patients regarding their surveillance appointments.
- Contact patients through at least two forms of communication to maximise attendance and reduce DNAs.

Digital & Data Responsibilities

- Support the coding of patients under active HCC surveillance and maintain up-to-date records of surveillance status
Team Support & Communication
- Provide a point of contact for patients on the HCC surveillance pathway, including appointment reminders and follow-up queries.
- Work closely with the Band 6 Early Detection Nurse and report any concerns regarding delays, deterioration, or clinical escalation needs.
- Participate in team meetings and support cross-cover for administrative colleagues to ensure continuity of service.

Essential criteria
- GCSEs in English and Maths (A-C) or equivalent
Desirable criteria
- NVQ Level 3/4 in Health/Social Care or Administration
Experience

- Experience in patient-facing or healthcare admin roles
- Experience using clinical databases or hospital IT systems
Desirable criteria
