Fairfield General Hospital is recruiting to a new and developing In-Patient Discharge Unit
If you are motivated, enthusiastic, and passionate about providing patient centred care, then ward 24 is for you. Ward 24 is a 24 bedded in-patient facility and operates alongside a Discharge Lounge. The ward's ethos is to prevent deterioration of patients whilst they are being cared for and to support and assist the integrated discharge team with enabling patients to go home from hospital rather than short/long term care. A home first approach is essential in this role. This is achieved by encouraging all patients to mobilise at every opportunity, challenging discharge pathways and carrying out Strength-Based Conversations.
Unfortunately, this role does not meet the criteria required to qualify for sponsorship.
