The Role

We are looking to recruit a part-time, Health Improvement Practitioner in the East of the county to assist our increasingly busy service to help improve the health outcomes for those people who are homeless and address the unique challenges they might face This is a permanent, part time role, 14.8 hours a week.

This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies.

This position will be subject to an enhanced criminal record disclosure check.

Working Pattern

This is a 2-day post, 14.8 hours per week and hours can be agreed with the team manager.

What You’ll Need To Succeed

We are seeking a compassionate, energetic person with experience supporting individuals facing health challenges related to homelessness.

  • An ability to motivate and inspire behaviour change in individuals who may initially be resistant to change in order to facilitate health improvement.
  • An ability to successfully deliver stop smoking interventions to individuals and advise on follow up support.
  • A strong work ethic and the ability to work both individually and as part of a team towards shared objectives.
  • Able to work flexibly across the week including a commitment to working some evenings and occasional weekends.
  • An ability to manage busy caseloads effectively
  • An ability to use evidence-based practice with a compassionate approach to tobacco dependency.
  • An ability to effectively collect and document information appropriately within a database.
