We have been piloting new ways to improve management of heart failure patients both within the acute setting and delivering innovative care within the community. There is now a great opportunity to join the team at the pivotal stage where we are testing the effectiveness of one of these pilots to go on and drive this service forward.
This post will allow us to expand and develop the services we offer. For you, this will mean an opportunity to be involved at the front of an innovative service, progressing and developing knowledge and skills in heart failure while working alongside committed and compassionate colleagues.
We have advertised the post as either a band 6 or 7 role, depending on experience. Support will be given to work towards heart failure specialist nurse competency framework set by the British Society of Heart Failure.
The post may become Permanent at the end of this Fixed Term/Secondment.

The post holder will be part of a team of experienced heart failure specialist nurses and be responsible for providing high quality expert care to patients with heart failure in York hospital and the Vale of York community setting.
The heart failure specialist nurse team deliver an out of hospital rapid access heart failure clinic based at a local health centre in central York,community nurse clinics in a variety of community setting across our Vale of York location and can support visiting patients in their own
homes. The team are also building a heart failure in-reach model which focuses on admission avoidance and early step down to community delivered care.
