If so this could be the perfect opportunity for you! Experience working as a PCN Dietitian is desirable but not essential, provided you have substantial experience in a specialist dietetic role.
The Role
As a PCN Dietitian, you will play a key role in proactive, patient-centred dietary care in primary care. You’ll work with GPs, nurses, pharmacists, social prescribers, health coaches, and other allied health professionals to provide evidence-based nutritional interventions that help patients manage and prevent long-term conditions.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
- To carry out dietetic assessment appropriate to role and experience, provide nutritional diagnosis and provide a dietetic treatment plan for patients / clients.
- To hold responsibility for own patient caseload including patients with complex presentations
- Maintain clinical records in line with local / Trust documentation guidelines
- To liaise with dietetic colleagues and other members of the multidisciplinary team as necessary e.g. Medical, nursing, catering and other health care professionals
- To undertake leadership and line management duties as required for the post
- To work within National and Trust policies / guidelines; and HCPC and BDA standards and codes of conduct
- To have an in depth understanding of national and local standards and able to monitor own practice against these
- To participate in bank holiday working and work flexible hours to meet the needs of the patients if required
