About The Role
Are you passionate about supporting people with multiple, complex needs? Do you thrive in a dynamic role where no two days are the same?
We are looking for a compassionate and proactive Hostel In-Reach Floating Support to join our East Kent team. You will work within an integrated healthcare framework to provide targeted, flexible support to adults (18+) with drug and alcohol issues, many of whom are rough sleeping or living in temporary or supported accommodation.
With a smaller caseload, you will have the time to build meaningful relationships and take a trauma-informed approach that meets people where they are. You will help clients access housing, health services, and wider support, working closely with partners across East Kent to coordinate care and improve outcomes.
This is a transitional role, helping individuals stabilise and move into permanent accommodation. It is ideal for someone who values person-centred care, understands the barriers faced by those with high support needs, and wants to make a real difference in the lives of some of the most marginalised people in our communities.
What The Role Involves…
- Support clients in B&Bs, hostels, and other temporary settings across East Kent
- Carry out assessments and develop personalised, trauma-informed care plans
- Provide one-to-one and group support based on individual need
- Coordinate with housing, health, criminal justice, and other partners to reduce harm and promote recovery
- Motivate clients to engage with services and progress toward stable housing, health, and wellbeing
The ideal candidate will have…
- Experience supporting adults with complex needs, including substance use and homelessness
- Understanding of trauma-informed, person-centred care
- Strong partnership working and communication skills
- Ability to manage a flexible caseload and work independently
- Willingness to travel across East Kent and occasionally work evenings
