Optimise is transforming how we deliver care across Cwm Taf Morgannwg UHB—and we’re looking for a passionate, people-focused individual to join our team as a Facilitator.
This Band 6 role is being offered as a secondment opportunity until 31 March 2026, based in RGH with travel required around sites, there is also some opportunities to work from home. (If you’re currently working within the NHS you must obtain permission from your current line manager prior to applying for this secondment.) The hours are Monday – Friday 9-5, with some weekend work required, but if this doesn’t work for you, we’re happy to discuss flexible options.

As an Optimise Facilitator, you’ll work closely with clinical teams, senior managers, and operational leaders to:
- Coach and support ward teams to improve effectiveness and efficiency, releasing more time to care.
- Champion patient-centred discharge planning, applying Optimal Hospital Flow principles from the point of admission.
- Inspire teams to focus on what matters most to patients and prevent deconditioning.
- Support the Optimise Lead Nurse in building QI capability across the organisation.
- Influence large-scale improvements aligned with the 6 Goals for Urgent and Emergency Care.
- Contribute to a national delivery plan that’s shaping care across Wales and beyond.
You will work as part of a supportive team who collaborate with clinicians, senior managers, and operational teams to drive service improvement and redesign. By promoting a culture of continuous improvement, you’ll challenge existing practices, eliminate inefficiencies, and enhance both patient flow and outcomes. Partnering with multidisciplinary teams, you’ll embed sustainable discharge pathways and apply national and local policies with a critical, respectful, and innovative approach.
